To get started, you need to accomplish an intake form.

We also require the following information:

• Complete our Intake Form Click here

• Documentation of COVID test within seven days of admission

• H&P history and physical within the one year

• Documentation of any incidents in the past six months

• List of current medications (doctors’ orders)

• Physician order (Form 3055)

• Complete a health exam with nurse (Form 3050)

• Metro lift application

• Proof of income (award letter from all source of income)
